Does the outline change the sprite size?

Yes — the canvas expands by the outline thickness on all sides. A 32×32 sprite with 1px outline becomes 34×34.

What thickness should I use?

Can I outline sprites with transparent backgrounds?

Yes, the outline is added around non-transparent pixels only. Transparent areas stay transparent.
